Our Approach

At FPH Wellness, we believe that optimal health is for everyone. For those of you continuously asking, "What is wrong with me?" and not getting an answer, our functional medicine dietitians, behavioral therapists, and physical therapists are here to help! Our integrative programs are designed to meet and support you wherever you are on your journey to better health.

Your employer offers FPH Wellness’ services in your benefit plan. To qualify for employee coverage of any of the following programs, your employer requires enrollment in the current benefit plan in addition to meeting a total point count of 45 using the following criteria:

  • BMI > 25 (15 points)

  • Fasting Blood Glucose >99 (15 points)

  • Diagnosis of any functional bowel disorder: abdominal pain, GERD, IBS, diarrhea, constipation, chronic bloating, altered bowel habits, SIBO, IBD (UC or Crohn's) (30 points)

  • Diagnosis of any mental health disorder including but not limited to anxiety, depression, personality disorder, PTSD, eating disorders (30 points)

  • Blood Pressure of > 120/80 (15 points)

  • Diagnosis of any musculoskeletal disorder including but not limited to osteoarthritis, arthritis, degenerative disk disorders, neck/back pain, scoliosis, joint derangement, TMJ (30 points)

  • Diagnosis of hyperlipidemia or hypercholesterolemia (15 points) TSH level of >2.5 (15 points)

  • Complimentary Discovery Call

    Schedule a complimentary discovery call to discuss your health concerns and goals. You will be prompted to complete the screening criteria questionnaire and answer a few short questions. This will allow our team understand your unique needs and determine the best approach for you.

  • Service Selection

    During your complimentary discovery call with one of our functional medicine dietitians, we will explore our service offerings with you, determine your program eligibility, and help you choose the path that best aligns with your health goals.

  • Initial Comprehensive Consultation

    Our team will begin the care process by creating a HIPAA compliant patient portal for you, sending you the required medical forms to complete, and then review your health history, dietary habits and patterns, past labs, stress, sleep patterns, and exercise habits to deeply understand your unique wellness needs. This session is 75 minutes.

  • Personalized Treatment Plan

    From the initial consultation, we craft a custom care plan targeting the root of your imbalances by incorporating functional nutrition, supplements, functional testing, movement, mind-body modalities, and lifestyle interventions for a holistic approach. We offer 45 minute bi-weekly sessions with your care team to ensure you have the necessary support to make meaningful change.

  • Education and Empowerment

    Our entire team provides you with knowledge and accountability, teaching you about food's impact on the body, practical tips for chronic conditions, and sustainable lifestyle skills to keep moving.

  • Collaboration and Support

    Receive continuous support and education. We track your progress, adjust your plan as needed, and offer strategies for overcoming challenges, ensuring consistent motivation and encouragement.
